Abstract
Background: Acute myocardial infarct (AMI) is an acute heart disease that occurs due to atherosclerosis with erosion, fissuring, and rupture of plaque in the coronary arteries. AMI requires fast and appropriate treatment to avoid serious complications. The prevalence of AMI cases in East Java itself is classified as dangerous, namely 1.6%, but the symptoms are often unknown. The clinical manifestation of AMI is usually sudden chest pain that continues at rest. Therapeutic management aims to reduce infarction and prevent ventricular remodeling. Therapy used in AMI where the supply of oxygen in the blood to the heart is inadequate so that nitrate vasodilator drugs are needed.
Methods: This research uses an observational research design, namely with no intervention in the sample used. Data was collected retrospectively because the data was obtained from the patient's medical records and then data processing was carried out. Meanwhile, using a descriptive research design was aimed at describing the pattern of use of nitrate vasodilators in acute myocardial infarct patients at regional public hospital dr. Iskak Tulungagung during the period January 2023-June 2023.
Result and Conclusions: The pattern of single vasodilator nitrate therapy was 66 patients (70%) and combination therapy was 28 patients (30%). The most commonly used single therapy was ISDN (3x5 mg) sl in 36 patients (49%), The combination nitrate therapy pattern most frequently received was ISDN (3x5 mg) sl + Nitroglycerin (1x200 mg) iv with 17 patients (53%). There are 11 patterns of switching therapy in acute myocardial infarct patients.
