Abstract
Background: Discharge planning is an essential part of nursing care to ensure that patients and their families understand the steps they need to take after leaving the hospital. Methods: This study employed a descriptive quantitative approach using observation and interviews. The subjects were 21 nurses in the inpatient ward of RSUD Dr. Saiful Anwar, selected using accidental sampling techniques. The instrument used was a checklist of the discharge planning SOP in the ward. Results: The majority of nurses performed the documentation aspect. However, health education, demonstrations, and evaluations of patient understanding were minimally conducted. Patients discharged without medical devices received brief education regarding follow-up schedules and medication consumption, while patients with medical devices were given more detailed education. Conclusion: The implementation of discharge planning was not fully in accordance with the standards. The main factor affecting this was the heavy workload of nurses,
resulting in suboptimal patient education. Improving workload management, providing continuous training for nurses, and adopting digital technology for discharge planning documentation were identified as crucial steps to support the optimization of discharge planning and prevent health complications leading to readmission.
