Abstract
Background: The ineffective implementation of discharge planning can be seen from the limited role of care, especially in the form of re-supervision, while patients actually need home health care, health counseling or education, and community services. Sometimes, patients even return within 24 to 48 hours and are then sent home again. Failure where discharge planning is not provided properly and is not archived adequately can pose a serious risk to the severity of the disease, potential threat to life, and physical disorders. Pakpahan stated that effective discharge planning is very meaningful in improving patient health, reducing length of stay and readmissions. Discharge planning also has an impact on mortality rates, health outcomes and financing.
Objective: The aim of this research is to understand the description of the implementation of discharge planning in the children's inpatient room at an East Java teaching hospital.
Method: The subjects of this research were nurses in pediatric inpatient care who carried out discharge planning. The research instrument uses a discharge planning procedure or SOP
Results: The results of the preparation aspect in the implementation of discharge planning, it was found that the preparation activities consisting of preparing the patient's status, mentioning the patient's problems, things that nurses need to teach the patient and family were in fact not carried out by nurses in the inpatient room. The results of the documentation in the implementation of discharge planning, it was found that the activities had been carried out well, recorded the name of the patient going home along with the collection of the RM (medical record), and recorded it clearly, easily read, signed with a clear name. When the client leaves the hospital the client must receive a summary brief health status.
