PENERAPAN LATIHAN SOSIALISASI BERTAHAP PADA KLIEN Tn. D 44 TAHUN DENGAN ISOLASI SOSIAL DI UPT REHABILITASI BINA LARAS PASURUAN

Background : This Final Scientific Work takes a case of social isolation related to the client’s reluctance to socialize so that it is difficult to be given education and therapy. The client also has a thought process disorder which causes the client to dislike gathering with others so that he is often isolates himself. Interventions that can be carried out are BHSP, helping clients know the causes of social isolation, the advantages and disadvantages of not connecting with others, and teaching clients to get acquainted gradually. The therapy provided is given in addition to communicating well.
Method : This research method uses a case study which includes assessment, data analysis, nursing intervention, implementation an evaluation. The author uses various data collection techniques such as anamnesis, observation, documentation studies and physical examinations to collect various data from patient Mr. D to complete the data, the author also interviews the patient and conducts periodic monitoring using the SDKI instrument.
Result : After providing nursing care for 10 days to Mr. D, the results of the signs and symptoms that have not changed such as feelings of wanting to be alone, withdrawing, and meaningless movements.
Discussion : Nursing problems of social isolation in clients include lack of support systems, insufficient in SP 2 and the recommendations needed are social skill training to improve interpersonal skills.
