Abstract

Background : Thought process disorders are a disturbance and inability or obstacle in the process of weighing (judgment), understanding memory and reasoning (reasoning). Thought process disorders can be influenced by somatic factors (brain disorders, fatigue). Psychotic factors (emotional, psychological disorders), and social factors (other social conditions). This case study aims to determine nursing care for Mrs. R with a nursing diagnosis of thought process disorder in the Gili Trawangan Room, RSUD DR. Saiful Anwar, Provinsi Jawa Timur.
Method : his research is a case study which is structured through a series of nursing care including: assessment, data analysis, nursing intervention, nursing implementation and evaluation of the assessment process and data collection carried out using various techniques, including: interviews with patients, observation and documentation.
Results : Based on the results of the assessment, the main nursing diagnosis was obtained for the patient Mrs. R is thought process disorders. Then the intervention given to the patient was SP 1-SP 4 thought process disorders.
Conclusion : After carrying out the nursing intervention for 3x24 hours on Mrs. R, then nursing problems such as impaired thought processes by administering SP 1-SP 4 are quite capable of supporting healing of thought processes.
