Abstract
Occupational Health and Safety is an important factor that needs to be considered by the company. Its application is influenced by human reliability in it. This study discusses the analysis of work accidents due to human error at PT. X which is engaged in manufacturing transportation equipment. Work accidents that occur include: gram scratched, burnt, and pinched by the press machine. This is due to the neglect of procedures and the use of PPE by operators. The methods used are Standardized Plant Analysis Risk Human Realibility Assessment (SPAR-H) and Human Error Assessment and Reduction Technique (HEART). The results showed that the largest HEP value in the SPAR-H method was 0.5558 on the steel work unit, 0.1668 on the minor assembly unit, 0.1668 on the sub assembly unit, 0.1668 on the carbody assembly unit, and 0.5558 on the tempering unit. While in the HEART method, the largest HEP value is 0.6164 for the steel work unit, 0.2153 for the minor assembly unit, 0.2153 for the sub assembly unit, 0.2153 for the carbody assembly unit, and 0.4285 for the tempering unit. Based on this assessment, several suggestions for improvement are given in the form of: training on the correct use of machines and tools, as well as emphasizing the use of PPE.
