Abstract

Caring for patients with heart failure in the intensive care unit (ICU) requires nurses to maintain a very high level of readiness and accuracy due to the highly fluctuating and rapidly changing clinical conditions of patients. This critical situation requires nurses to integrate technical medical skills with emotional sensitivity in providing nursing care. This study aimed to explore in depth the experiences of ICU nurses in caring for patients with heart failure at Universitas Muhammadiyah Malang Hospital. Methods: This study employed a qualitative approach with a case study design. The participants consisted of three ICU nurses selected using purposive sampling. Data were collected through semi-structured interviews conducted twice for each participant to obtain in-depth information. The interviews were carried out in the ICU setting and scheduled according to the participants’ available time to avoid disrupting patient care. The collected data were analyzed using thematic analysis to identify meaningful patterns within the participants’ narratives. Data trustworthiness was ensured through source triangulation to maintain consistency of the information obtained. Results: Five main themes were identified: (1) nurses’ readiness in anticipating changes in patients’ conditions; (2) the importance of team collaboration in influencing the speed of interventions; (3) vigilance and rapid responses to unexpected clinical risks; (4) prioritizing patient comfort and stability; and (5) the implementation of open communication and emotional support for patients’ families. Conclusion: The experiences of ICU nurses indicate that successful care for patients with heart failure highly depends on a combination of technical competence, effective communication, and solid team coordination. Continuous support and intensive training are needed to further improve service quality and patient safety in critical care units.
