Abstract
Background: Non-adherence to antihypertensive medication can accelerate the progression of hypertension and cause damage to vital organs, such as the heart and kidneys. In the elderly, cognitive impairment exacerbates this condition by making it difficult to absorb new information, follow medical instructions, and maintain consistent treatment. Consequently, compliance decreases, blood pressure control is impaired, and the risk of complications increases.
Methods: A quantitative design with a cross-sectional approach was used at the Kendalsari Community Health Center. A sample of 179 elderly people who met the inclusion criteria was obtained through accidental sampling. Data were collected through direct interviews with respondents using two instruments: the MoCA-INA and the MMAS-8. Statistical analysis was performed using the Spearman correlation test.
Results: The study results showed that the majority of older adults with hypertension experienced mild cognitive impairment, accounting for 129 participants (83.2%), and had a low level of medication adherence, accounting for 76 participants (42.5%). The results of the Spearman’s Rho correlation test showed a correlation coefficient (rs) of 0.357, indicating a significant relationship between cognitive function and medication adherence among older adults with hypertension, with a significance value of p = 0.001 (p < 0.05).
Conclusion: Cognitive function plays a role in medication adherence in elderly hypertensive patients. Healthcare providers need to consider cognitive status in medication education and management to improve adherence and optimize hypertension control.
