Abstract

Background: Medication errors are a global issue often caused by inaccurate patient identification. In the Semeru Ward of RSUD Saiful Anwar Malang, initial observations revealed that medication boxes only listed bed numbers without complete patient identities, posing a risk of Near-Miss Events (NME). Objective: To evaluate the effectiveness of using identity labels on medication boxes to support accurate patient identification and nurse compliance. Methods: This is a quantitative study with a pre-experimental design (one-group pretest–posttest). The research subjects included 25 medication boxes and nurses in the Semeru Ward. Data were collected through direct observation before and after an intervention involving the addition of identity labels (Name and Medical Record Number). Results: The average nurse compliance score increased significantly from 1 (pretest) to 5 (posttest). The effectiveness of identification on medication boxes also improved from the "Ineffective" category to "Moderately Effective". Conclusion: The intervention of providing identity labels on medication boxes effectively improves patient identification accuracy and nurse compliance, although more permanent standardization of labels is required.
