Abstract

Background : Non-hemorrhagic stroke may lead to complications such as sleep
pattern disturbances, which can reduce quality of life and hinder recovery.
Appropriate nursing care is required to improve sleep quality through safe and
easily applicable non-pharmacological interventions.
Methods : This study used a case study design with a nursing process
approach, including assessment, nursing diagnosis, planning, implementation, and
evaluation. The subject was Mr. D, a post–non-hemorrhagic stroke patient
experiencing disturbed sleep patterns. Nursing care was conducted over five days
with morning home visits. The intervention consisted of sleep support through
education and demonstration of a 30° head-up position and murottal therapy,
which were performed independently by the patient and family at night. Sleep
quality was evaluated using the Pittsburgh Sleep Quality Index (PSQI).
Results : The nursing care results showed an improvement in the patient’s
sleep quality. Subjectively, the patient reported longer sleep duration, reduced
nighttime awakenings, and feeling more refreshed upon waking. Objectively, The
PSQI score decreased from 12 at the beginning of nursing care to 4 at the end of
nursing care measured within 5 days, indicating an improvement in sleep quality
from poor to good.
Discussion : Sleep support interventions combining a 30° head-up position and
murottal therapy provided physical and psychological relaxation, facilitating sleep
initiation and maintenance. The educational approach also enhanced patient and
family independence in sustaining healthy sleep habits.
