Abstract
Background: Gout arthritis is an inflammatory joint disease caused by the accumulation of monosodium urate crystals, resulting in severe pain, swelling, and activity limitations that can reduce patients’ quality of life. In addition to pharmacological therapy, some patients utilize non-pharmacological approaches in the form of traditional medicine, one of which is bay leaf decoction. Pain management in patients with gout arthritis is influenced not only by medical treatment but also by patients’ subjective experiences in choosing and undergoing such therapy. Objective: This study aimed to explore the experiences of patients with gout arthritis in using bay leaf decoction to reduce pain. Methods: This study employed a qualitative method with a case study approach and content analysis. Data were collected through in-depth interviews, observation, and documentation involving three participants: one patient with gout arthritis as the main participant and two family members as supporting participants. Data trustworthiness was strengthened through source triangulation. Results: The findings showed that pain experiences in patients with gout arthritis based on the PQRST approach included pain triggered by high-purine food consumption, heavy physical activity, and frequent occurrence at night (Provocation). Pain was described as stabbing, burning, and throbbing sensations in the joints, particularly in the extremities, with moderate to severe intensity and a recurrent pattern (Quality, Region, Severity, Time). The use of bay leaf decoction was perceived to reduce pain intensity, decrease swelling, and lower the frequency of recurrence. Conclusion: Pain management in gout arthritis is individual in nature and influenced by physical, psychological, social, and cultural factors. The use of bay leaf decoction is perceived as a safe and beneficial complementary therapy for reducing pain, highlighting the need for a holistic and patient-centered nursing approach.
