Abstract

Type 2 Diabetes Mellitus (T2DM) is a chronic metabolic disease characterized by unstable blood glucose levels and continuously increasing global prevalence. In 2021, approximately 537 million individuals aged 20-79 years were living with diabetes worldwide, and this number is projected to rise to 643 million by 2030. Chronic hyperglicemia in T2DM not only increases the risk of microvascular and macrovascular complications but also contributes to cognitive decline. Long-term therapeutic success in T2DM is strongly influenced by medication adherence, as poor adherence may worsen glycemic control and accelerate cognitive impairment. This study aimed to examine the relationship between medication adherence and cognitive function among patients with T2DM attending the internal Medicine Outpatient Clinic of Universitas Muhammadiyah Malang Hospital. This quantitative study employed a cross-sectional design and involved 101 respondents. Medication adherence was assessed using the Morisky Medication Adherence Scale-8 (MMAS-8), while cognitive function was measured using the Mini-Mental State Examination (MMSE). The results showed that 80.2% of respondents had low medication adherence, 13.9% had moderate adherence, and 5.9% had high adherence. Regarding cognitive function, 26.7% of respondents were classified as normal, 42.6% had mild to moderate cognitive impairment, and 30.7% had severe cognitive impairment. Spearman correlation analysis revealed a significant relationship between medication adherence and cognitive function (p = 0.000), with a positive direction and a moderate correlation strength (r = 0.460). In conclusion, higher medication adherence is associated with better cognitive function. Strengthening interventions to improve medication adherence is essential to prevent cognitive decline in patients with T2DM.
