PENERAPAN 3S (SDKI, SLKI & SIKI) DALAM PENDOKUMENTASIAN ASUHAN KEPERAWATAN DI RUANG PANGANDARAN RUMAH SAKIT Dr. SAIFUL ANWAR

Background: The implementation of nursing care documentation according to 3S (SDKI, SLKI, SIKI) is rarely applied as client documentation in various hospitals in Indonesia, this is because many nurses have not attended socialization and training on 3S (SDKI, SLKI, SIKI). Nurses in Indonesia are still accustomed to documenting clients using NANDA NIC NOC-based nursing care standards so that many nurses still have difficulty adapting to documenting clients using 3S nursing care standards. Therefore, researchers are interested in conducting counseling related to the
implementation of dormitories based on the 3S format (SDKI, SLKI and SIKI) as dormitory documentation in the Pangandaran room of Dr. Saiful Anwar Regional
Hospital for 3 weeks.

Research Methods: Interview: Researchers collected data by conducting interviews with all nurses using a nurse knowledge questionnaire related to 3S to determine the level of nurses' knowledge regarding 3S-based nursing documentation.

Counseling:Researchers conducted direct counseling to 3 nursing staff and all 10 implementing nurses related to 3S-based nursing care documentation.

Documentation: Researchers documented the results of nurses' nursing care to determine nurses' compliance in documenting 3S-based nursing care on the client's CPPT sheet and SimRS.

Results: In the socialization evaluation stage which was carried out for 3 days, the
researcher obtained significant results that the implementation of 3S-based nursing care documentation in the Pangandaran Room was actively participated by 13 nurses including 3 room team heads, based on the observation results it was found that 3S based nursing care documentation had been implemented 100% on 35 clients and had been implemented as client documentation in SimRs.

Discussion: Providing counseling related to 3S can be a strategy to increase knowledge for nurses, besides that, there is also the level of education and length of service which are supporting factors in increasing the level of knowledge of nurses.
