Abstract

Hypertension is a chronic disease commonly experienced by the elderly and can lead to complications such as stroke and heart failure. One factor that worsens this condition is the habit of smoking. However, many older adults continue to smoke due to comfort, long-standing habits, and environmental influences. Mr. S is one of the elderly individuals who continues to smoke despite being diagnosed with hypertension. To describe the experience of Mr. S in living his daily life as an active smoker with hypertension. A qualitative case study using in-depth interviews with three participants and source triangulation. Five main themes were identified related to the initiation and experimentation with cigarettes, addiction to smoking and the difficulty of quitting the habit, the impact of smoking severity on hypertension, adherence to antihypertensive therapy and the use of herbal products, and emotional support from family in reducing smoking behavior. Although older adults understand the harmful effects of smoking on hypertension, the habit is still maintained due to psychological and environmental factors, thereby worsening the hypertensive condition, which highlights the importance of nurses’ roles in providing continuous health education, strengthening motivation for smoking cessation, and consistently involving families through a holistic nursing approach.
