Abstract

Background: Bipolar disorder is a mental disorder characterized by extreme mood swings
and can be accompanied by psychotic symptoms such as visual hallucinations. This
condition poses a safety risk to the patient and others. Treatment for hallucinations can be
carried out pharmacologically or non-pharmacologically, with classical music therapy
being an effective treatment because it has a relaxing effect, reduces stress, and helps with
emotional control.
Method: This research design was a case study with a nursing care approach. The subject
was a 20-year-old female patient experiencing visual hallucinations due to an acute
episode of bipolar disorder (F31). The intervention was carried out over five days using
Implementation Strategies (SP) 1-4 for hallucinations combined with classical music
therapy, lasting 15-20 minutes per session. Data were collected through observation,
interviews, and documentation of the patient's progress using the Indonesian Nursing
Outcome Standards (SLKI).
Results: After being administered SPTK in accordance with the nursing diagnosis and
established care plan, on the first day the patient exhibited six symptoms of visual
hallucinations. After five days of treatment, these symptoms gradually decreased, leaving
only one symptom, pacing, remaining. On days 4 and 5, an additional intervention in the
form of classical music therapy was administered to the patient.
Conclusion: The effectiveness of classical music therapy as a relaxation method cannot be
definitively evaluated because the patient was also receiving pharmacological therapy and
SPTK at the same time. This introduced bias, making it difficult to determine whether the
reduction in visual hallucination frequency was more influenced by music therapy,
medication, or a combination of the two.
