PENERAPAN TERAPI ORIENTASI REALITA PADA PASIEN WAHAM DI UPT REHABILITAS SOSIAL PASURUAN KARYA ILMIAH AKHIR NERS (KIAN)

Background: Mental disorders are one of the serious challenges in the world of
mental health that can affect various aspects of an individual's life. One form of
mental disorder that is quite complex is delusion. Delusions are thoughts or beliefs
that are inconsistent with reality and cannot be proven objectively. Patients with
delusional disorders experience distortions in their understanding and judgment of
reality, which can lead to anxiety, social isolation, and difficulties in carrying out
daily routines.
Purpose: This study aims to evaluate the results of nursing care for clients with
delusional disorder
Method: In this study using a descriptive method in the form of a case study where
this method focuses attention on one patient or a special object with a diagnosis of
Schizophrenia who is experiencing a disorder and also uses methods with
interviews, observation and documentation studies which include Assessment,
Nursing Diagnosis, Planning, Implementation and Evaluation
Results: Based on the diagnosis obtained, the evaluation results show that with the
application of reality orientation therapy, delusional patients can control their
delusions well even though in the middle of the interview sometimes delusions still
arise which continue to be talked about.
Conclusion: In this study it was concluded that reality orientation therapy was very
influential in reducing symptoms for clients with thought process disorders:
