Abstract
Background: Liver cirrhosis is a chronic and irreversible disease characterized by liver cell damage, fibrosis, and the formation of regenerative nodules. Clinical manifestations of cirrhosis include portal hypertension, hematemesis melena, ascites, SBP, esophageal varices, hepatic encephalopathy, hepatorenal syndrome, and gastrointestinal bleeding. The therapy given for hematemesis and melena is tranexamic acid and K vitamin. Tranexamic acid acts as an antifibrinolytic by inhibiting plasminogen activation, thereby preventing fibrinolysis, while K vitamin functions in the blood clotting process through the activation of coagulation factors.
Objective: To determine the pattern of tranexamic acid and K vitamin use in hepatic cirrhosis with hematemesis melena patients, including dosage, route, frequency, duration of administration, combination and switching patterns at Kanjuruhan Regional General Hospital in Malang.
Method: This study is a descriptive retrospective study using medical records of patients with hepatic cirrhosis and hematemesis melena at Kanjuruhan Regional General Hospital in Malang, from January 2022 to December 2024.
Results and Conclusions: The single treatment regimen used in all patients, the most single regimen is tranexamic acid (1 x 500 mg) iv and K vitamin (1 x 10 mg) iv in 12 patients (29%). Switching regimen in 11 patients (21%), duration of tranexamic acid and K vitamin was 3-7 days in 10 patients (48%).
