Abstract

Introduction: Schizophrenia is a severe and chronic mental disorder that can affect 20 million people worldwide. Schizophrenia is a chronic, serious and disabling disease, 1 in 5 people experience mental disorders, meaning that 20% of the population in Indonesia has the potential to experience mental disorders. Residual schizophrenia experiences a decline or even a complete inability to interact or experiences social isolation.
Method: The research method used in this research is using an appropriate case study
with the nursing process consisting of assessment, data analysis, nursing intervention, implementation and evaluation.
Results: From the results of data analysis, 5 nursing diagnoses were obtained, namely social isolation, self-care deficit, verbal communication disorders, memory disorders, and thought process disorders. Of the 5 nursing diagnoses that emerged, the author focused on taking the 2 main actual nursing diagnoses, namely social isolation and self-care deficit.
Conclusion: From the application of SPTK (Nursing Action Implementation Strategy) in social isolation, clients are able to interact in a real way, clients are also able to bathe well and wear neat, unlined clothes.
