Abstract
Background: This thought process disorder is a positive symptom of schizophrenia and usually people who have these symptoms will do things that are not in accordance with the type of delusion, namely by having a high sense of suspicion of themselves and others, feeling great power, feeling extraordinary strength,. The prevalence of mental disorders worldwide according to WHO data, (World Health Organization) in 2019, there were 20 million people experiencing schizophrenia. Mental disorders are a serious health problem because the number of diseases continues to increase, including schizophrenia which affects the thinking process for sufferers. This scientific work aims to analyze mental nursing care with the problem of disturbed thought processes. Methods: The research method used in the preparation of this Final Scientific Paper uses a case study report. The author follows the scientific method in accordance with the rules of the nursing process which includes: assessment, data analysis, nursing intervention, implementation and evaluation. In the assessment process, the author uses data collection techniques such as history taking, observation, interviews and physical examination to collect some data. Results: After performing nursing care, the author found nursing problems of mental disorders to carry out nursing action implementation strategies. Discussion: Providing reality orientation therapy is an intervention in increasing awareness of self-identity, time and environment. Reality orientation interventions become non-pharmacological treatment in patients with delusional disorders. This reality orientation uses the Strategy for Implementing Nursing Actions (SPTK) waham SP 1-4. CBT (Cognitive Behavior Therapy) therapy is effective to be given to schizophrenia patients with delusions, which can produce better social and occupational functions. This study is similar to previous journal studies which explain that cognitive therapy (CBT) has many positive benefits for patients with delusions both in reality orientation and anxiety levels.
