Abstract

PT. Literindo Berkah Karya is a company engaged in the printing industry that encounters product defects during the production process. In identifying the causes of defects and analyzing quality control, this research uses the Six Sigma method. Then it provides improvement proposals using the Failure Mode Effect Analysis (FMEA) method. It was identified that there are 4 types of products experiencing defects, including improper book cutting defects, blurred colors, inappropriate string lengths, and weak glue. The improper book cutting defect is the highest defect, reaching 40.24% with a total of 33 products. Recommendations for improvement to reduce defects include implementing clear Standard Operating Procedures (SOP) for each production process, additional training for workers, and ensuring that the final products meet the company's quality standards.
