Abstract

Chronic kidney failure leads to a decline in kidney function, affecting the body's fluid balance and metabolism. Patients with chronic kidney failure must undergo regular hemodialysis to survive. This condition impacts not only their physical health but also their emotional well-being, making it difficult for patients to maintain an optimistic attitude in daily life. Reality therapy employs the WDEP approach (Want, Doing, Evaluation, Planning), which helps patients recognize their needs and desires, assess their behaviors, and plan changes to achieve better goals. This study aims to evaluate the effectiveness of reality therapy in enhancing optimism among chronic kidney failure patients undergoing hemodialysis. The research utilizes a small-N design with three subjects undergoing routine hemodialysis. Data were collected through interviews, self-reports, and the LOT-R (Life Orientation Test-Revised) optimism scale to measure changes in optimism levels before and after therapy. The results indicate a significant increase in optimism following therapy. Before the intervention, subjects felt pessimistic about the future, viewed problems as disasters, and blamed themselves for their situations. However, after the intervention, subjects began to view the future more positively, believing that difficulties were temporary, and reduced their tendency to self-blame. The conclusion of this study is that reality therapy is effective in enhancing optimism among chronic kidney failure patients, making it a valuable psychological intervention.
