Abstract

Patients with kidney failure undergoing hemodialysis often experience significant
stress due to physical and emotional burdens. This condition lowers their quality of life
and requires effective interventions to reduce stress. Reality Therapy, which focuses
on understanding personal needs and action planning, is used to help patients manage
stress. This study aims to evaluate the effectiveness of Reality Therapy in reducing
stress in hemodialysis patients using a small-n design, which allows for an in-depth
exploration of individual changes. Through the WDEP stages (Wants, Doing,
Evaluation, Planning), analysis was conducted on three subjects, each showing unique
responses based on their needs. Subject 1 experienced stress reduction through
increased physical activity, Subject 2 through strengthened social relationships, and
Subject 3 through emotional management via journaling. Intra-case results showed
significant changes in stress management for each subject, while inter-case analysis
revealed varied response patterns based on the specific needs of each individual. This
study highlights the strength of the small-n design in capturing individual variations
and changes that may not be revealed in large-scale research, offering a richer
understanding of the effectiveness of Reality Therapy interventions.
