Abstract
Background: Heart failure is a progressive syndrome resulting from structural or functional cardiac dysfunction, generally triggered by impaired myocardial contractility and relaxation. In the management of heart failure, angiotensin receptor blockers (ARB) work by inhibiting the angiotensin II type 1 receptor, thereby inhibiting ventricular remodeling and reducing vasoconstriction and fluid retention.
Objective: To determine the pattern of ARB use in heart failure patients, including type, dose, route, frequency, duration, combination pattern and switch at District Hospital dr. Mohamad Soewandhie Surabaya.
Methods: The study was a descriptive observational study with retrospective data and analyzed descriptively to show the pattern of ARB use.
Results and Conclusions: No single pattern was found, but rather a combination of 2 (11%), combination of 3 (35%), combination of 4 (24%), combination of 5 (7%). The most common combination pattern 2 is Candesartan (1x8mg) po + Bisoprolol (1x1.25mg) po 6 patterns (43%), combination 3 Candesartan (1x8mg) po + Bisoprolol (1x1.25mg) po + Spironolactone (1x25mg) po 10 patterns (22%), combination 4 Candesartan (1x16mg) po + Bisoprolol (1x2.5mg) po + Spironolactone (1x25mg) po + Nifedipine (1x30mg) po 7 patterns (22%), there are 8 combination patterns of 5 drugs each by 1 patient. There are 29 switch patterns. The longest duration of use was in the range of 3 – 5 days for 30 patients (48%).
