Abstract
Background: Acute myocardial infarction (AMI) is a leading cause of cardiovascular morbidity and mortality. A key strategy in the management of AMI is the use of anticoagulants to prevent thrombus formation and dissemination. LMWH, in particular, is frequently used to prevent thrombus progression and thrombotic complications. The use of LMWH in AMI therapy aims to optimize reperfusion, reduce the risk of recurrent thrombotic events, and improve patient clinical prognosis. Therefore, evaluating LMWH usage patterns in healthcare facilities is crucial to ensure compliance with therapy standards.
Objective: To evaluate the pattern of LMWH use in hospitalized patients with a final diagnosis of AMI at RSUD dr. Mohamad Soewandhie Surabaya and to assess its appropriateness based on current clinical guidelines.
Methods: This study employed a retrospective descriptive design by reviewing medical records of AMI patients hospitalized at RSUD dr. Mohamad Soewandhie Surabaya from March 1, 2025, to August 31, 2025.
Results and Conclusion: All patients received LMWH as monotherapy, with enoxaparin (2×60 mg) subcutaneously being the most frequently used regimen (62%), followed by fondaparinux (1×2.5 mg) (5%). Four switching patterns were identified in 22 patients (19%), and the most common duration of therapy was 3–6 days (57%).
