Abstract
Construction projects face various challenges, one of which is occupational accidents. Work accidents in construction projects can be caused by several factors, including a lack of workers’ understanding of work methods, improper use of materials and equipment, unfavorable working environment conditions, and inadequate physical conditions of workers. This study aims to analyze the implementation of Occupational Health and Safety (OHS) at the Eco Green Church Bethany Yestoya Malang construction project using the Hazard Identification and Risk Assessment (HIRA) and Fault Tree Analysis (FTA) methods. The HIRA method is applied to identify potential hazards and assess risk levels based on the likelihood and consequences of occurrence, while the FTA method is used to trace the root causes of work accidents through the analysis of top events and contributing factors. Risk variables obtained from the HIRA method are further analyzed using FTA to determine the basic events that cause occupational accidents. Research data were collected through field observations, interviews, and questionnaires as primary data, while secondary data were obtained from project documents and literature studies. The results show that there is 1 variable categorized as low risk, 27 variables as medium risk, and 4 variables as high risk, including the risk of workers falling from heights, being scratched by formwork materials, being struck by reinforcing steel, and electrical short circuits. Furthermore, four main factors causing occupational accidents were identified, namely human factors, management factors, environmental factors, and technical factors, with human factors being the most dominant. This study produces risk control recommendations that can minimize work accidents, increase OHS awareness, and support the creation of a safe and productive working environment in construction projects.
