Abstract
Background : Cirrhosis is a chronic liver disease characterized by liver injury,
fibrosis, and nodular regeneration. According to data from the World Health
Organization (WHO) in 2016, approximately 51.1% of men and 27.1% of women
per 100,000 population died from cirrhosis. The prevalence of death in South Asia
and Southeast Asia due to cirrhosis is 44.9%. Spironolactone is a first-line therapy
that belongs to the aldosterone antagonist class by stimulating sodium reabsorption
and potassium excretion through the activation of the Na⁺/K⁺ pump in distal tubule
epithelial cells.
Objective : To determine the therapeutic patterns of spironolactone, including
dosage regimen, frequency, route of administration, duration of use, combination
patterns, and switching patterns in patients with liver cirrhosis complicated by
ascites.
Methods : This study was observational in namely with data collected
retrospectively using descriptive research at Kanjuruhan Malang Hospital.
Results and Conclusions : Use of spironolactone therapy profiles: single agent
(53%), two combination therapy (47%). The most common used single regimen is
spironolactone (1x25mg) po, and the most common combination regimen is
spironolactone (2x100 mg) po + furosemide (1x40 mg) iv. A switch regimen 10
patients (33%).
