Abstract
This study aimed to evaluate the potential for drug interactions in outpatient prescriptions for Diabetes Mellitus (DM) patients with hypertension at the Wagir Community Health Center during January–March 2022. DM and hypertension are two chronic diseases that often coexist and require long-term combination therapy, which carries a high risk of drug interactions. This study used a retrospective descriptive method with secondary data collection from the medical records of patients who met the inclusion criteria. A total of 129 patients were analyzed based on their demographic profile, type of antidiabetic and antihypertensive therapy, and potential drug interactions. The results showed that the majority of patients were women aged 51–60 years. The most frequently prescribed antidiabetic drug was metformin, while the most commonly prescribed antihypertensive drug was amlodipine. Potential drug interactions were found in 88% of patients, with the most frequent interactions between metformin and amlodipine being pharmacodynamic and moderate in severity. The most dominant interaction mechanism was pharmacodynamic. This study emphasizes the importance of therapeutic monitoring and evaluating the rationality of drug use to avoid adverse side effects and improve treatment effectiveness. Patient education and interprofessional collaboration in pharmaceutical services are also needed.
