Abstract
Background: Reliable assessment of patient problems is essential. Guidelines for their use are also
crucial for assessment instruments. Standardized assessment procedures are crucial because they
can reduce unwanted bias.
Methods: This study uses a qualitative approach with a case study design. Data were obtained
through in-depth interviews, participant observation, and documentation in the Gili Trawangan
inpatient room.
Results: The initial assessment used the RUFA scale. The client was categorized as intensive phase
III (acute phase). Therefore, the accuracy of the assessment as a diagnostic determination will
influence the subsequent intervention plan.
Discussion: This study shows that using RUFA as a supporting objective data to determine the
patient's diagnostic category. Initial patient assessment is important as a determination of
appropriate intervention in patients. In patients with this risk of recurrent events, RUFA helps analyze
the status of the client.
