Islamic Religious Education Study Program, FAI, University of Muhammadiyah Malang. Supervisor: (1) Dr. H. M. Nurul Humaidi, M.Ag. (2) H.N Taufik, M.Ag. Keywords: formation of religious behavior, adolescents, and families. The formation of religious behavior is the formation of every action in accordance with religious values and rules. This study aims to determine the efforts to shape the religious behavior of adolescent children in the family, as well as to determine the impact of the formation of religious behavior of children in the family on its implications in school institutions. This study uses a descriptive qualitative approach with an in-depth study in order to obtain complete and detailed data, with the type of research used is a case study. Data were collected through observation, interview and documentation techniques. The data analysis technique used is through data reduction, data presentation and conclusion drawing. The results of the study describe that efforts to form the religious behavior of adolescent children in the family are by providing education, providing guidance, providing coaching, and providing examples. Besides the efforts made by the family, there are several factors that influence the unfavorable behavior of teenagers, namely environmental factors, friendships and lifestyle. So the impact of the formation of religious behavior in the family on the religious behavior of students at SMA Muhammadiyah 1 Babat is that students have aqidah values, moral values, and worship values that have been instilled by the family. The implementation of students' moral values to all school members, especially teachers, including a gentle and polite attitude, helping each other in goodness, inviting goodness and forbidding from bad, respecting and fulfilling the rights of fellow human beings, as well as religious values that consistent.
