Abstract

Background: PCC is a health care approach that focuses on patients, their families and their communities. PCC respects patients' rights and values, and involves patients and their families in decisions about their care. Communication of various information regarding patient progress between health professionals in hospitals is a fundamental component in patient care, inaccurate information can have a serious impact on patients, almost 70% of sentinel events, namely events that result in death or serious injury in hospitals are caused by poor communication Objective: To find out how to implement interprofessional collaboration in the implementation of patient centered care. Method: This literature review was carried out using the narrative review method which is an integrated analysis. Results: Data search was carried out 9 journals were obtained. Based on the research journals analyzed, it was found that good communication between various health workers can improve patient outcomes. Conclusion: Implementing patient centered care in an interprofessional collaborative manner can improve patient outcomes and provide more positive relationships between health workers.
