Abstract

Backgroud: Diabetes mellitus type 2 is diabetes that results from progessive damage to pancreatic β cells which are motivated by insulin deficiency and insulin resistance. Management is done by using oral antidiabetic drugs and insulin and changing lifestyles to be able to reduce the incidence of type II diabetes mellitus. Insulin in diabetes mellitus is used if blood glucose cannot be controlled only with diet and antihyperglycemic acid. Insulin is needed when the HbA1c level> 9% with a condition of metabolic decompensation, severe hyperglycemia accompanied by ketosis, fails with oral antihyperglycemia drug therapy. Generally insulin used is basal insulin to prevent hyperglycemia during fasting, and prandial insulin to prevent hyperglycemia after eating. Objective: To find out the patterns of the use of Insulin in diabetes mellitus type 2 patients on inpatients of Dr. Iskak Tulungagung Hospital for the period of Januari-March 2019. Method: Retropective discriptive in diabetes mellitus type 2 at Dr.Iskak Hospital of Tulungagung from Januari-March 2019 Result and Conclusions: The use of single insulin is 121 patients (56%), the most is long acting 1x10IU sc (11%). Two combinations of insulin was 79 patients (37%), the most being aspart (Novorapid®) sc (3x6IU) with detemir (Levemir®) sc (1x10 IU) (6%). A combination of insulin with OAD each of 15 patients has a different patterns (7%). The switching patterns of therapy were 47 patients.
