Abstract

Background : Discharge planning, or the process of planning for a patient's return home, aims to prepare healthcare and social services before and after a patient is discharged. The role of nurses in discharge planning is of significant importance. As a nurse, they are responsible for all forms of nursing care provided to the patients, including playing a central role in patient discharge planning. The ineffective implementation of discharge planning can be seen from the limited role of care, so that patients often return to the hospital with the same disease or the emergence of more serious disease complications. The aim of this research is to determine the implementation of discharge planning in the internal medicine inpatient installation at Kanjuruhan Regional Hospital, Malang Regency.
Methods : This research uses a descriptive research design. The sampling technique used was Total Sampling, involving all 14 nurses in the Internal Medicine Inpatient Installation at Kanjuruhan Regional Hospital, Malang, who carried out discharge planning. In terms of data collection methods, standard operational observation sheets for discharge planning procedures and interviews with the head of the internal medicine inpatient installation at Kanjuruhan Hospital, Malang were used.
Result : As a result of the implementation, it was found that the activity most often carried out was the implementing nurse 1 expressing gratitude when carrying out discharge planning with a total of 10 activities or 38.5%. The activity that was most often not carried out was the Executive Nurse delivering health education and then carrying out demonstrations and re-demonstrations totaling 19 activities or 73%. The results of the documentation on the implementation of discharge planning, it was found that the activities that were most often carried out were the discharge planning document, recording the name of the discharged patient along with collecting the RM (medical record), and recording it clearly, easily to read, signing it with a clear name with a total of 24 activities or 92.3%.
Conclusion : Several challenges were also encountered during the process of discharge planning, such as the limited availability of leaflets provided by the hospital. In the future, it would be beneficial to provide leaflets so that patients and their families can review the given education. Nurses also provide health education to families with the aim of enhancing their understanding of the patient's condition and imparting knowledge that can expedite the patient's healing process.
