Abstract

Background : Schizophrenia is an abnormality characterized by positive symptoms of delusion, abnormal perception (hallucination), unorganized speech and behavior also negative symptoms including social self-withdrawal, apathy and lack of speech. The basic treatment for schizophrenic patients is antipsychotic. Clozapine is the only approved antipsychotic for drug-resistant schizophrenic patients whose response rate is about 30% to 50%. Another positive influence of clozapine is used as a therapy for psychotic patients who have suicidal tendencies. Objectives : This study is aimed to examine the pattern of clozapine antipsychotic usage including the dosage, frequency, and usage route also the therapy length for schizophrenic patients. Methods : This study is an observational research using retrospective method on schizophrenic patients from October to December 2017 at Dr. Radjiman Wediodiningrat Mental Hospital, Lawang, Malang. Results and Conclusions : Single use on Clozapine (1x25 mg and 2x25mg)PO is only given to 3 outpatients. For them (outpatients), the 3-combination mostly used are antipsychotic Clozapine (1x25mg)PO, Risperidone (2x2mg), and one anticholinergic trihexyphenidyl HCl (2x2mg)PO, in which it is given to 9 patients (12%). For hospitalized patients, the 2-combination mostly used are Clozapine (1x25mg)PO and Risperidone (2x2mg)PO for 4 patients (80%), also 4-combination of Clozapine (1x25mg)PO, Risperidone (2x2mg)PO, haloperidol (1x5mg/ml)IM and diazepam (1x5mg/ml)IM given to 4 patients (80%). The switch pattern usages on outpatients are 28 patterns, while on hospitalized patients is only 1 pattern.
