Abstract

Background : Stroke is the major cause of mortality and morbidity in the world. In Indonesia, patients with stroke reached 8.3 ‰ of population. Patients with stroke can cause a decrease in innate immunity and adaptive immunity which is an immune response that predisposes to work the first step to infection, particularly pneumonia and urinary tract infections. The infection is associated with adverse patient outcomes and increased health care costs. Objectives : To determine the pattern of use of quinolones antibiotics, including the suitability of dose, route, interval, duration of antibiotic quinolones, other singular antibiotics, a combination or replacement antibiotics in hemorrhagic stroke patients hospitalized with infection in RSU Dr. Saiful Anwar Malang. Method : This study was an observational study, which employed a retrospective descriptive analysis period 1 January to 31 December 2012 to hemorrhagic stroke patients’s medical record who were diagnosed and receiving quinolones antibiotic therapy. Results and Conclusions : 45 hemorrhagic stroke patients with infections consisted of 28 women (62%) men and 17 (38%) received antibiotic therapy that is mostly consisted of quinolones ciprofloxacin 39 patients (85%) intravenously 43 ( 96%) at a dose of oral 2x200mg and 2x400mg and 2 (4%) at a dose of 2x500 mg, as well as the highest singlular quinolones antibiotic ciprofloxacin consisted of 10 patients (84%) and a combination of two antibiotics quinolones and other antibiotics are ciprofloxacin + ceftriaxone 26 patients (74%). The most widely used therapy is empirical therapy 43 (96%), 1 + patients received definitive empirical therapy and 1 other patient received definitive therapy based on the results of microbiological culture test. Type of infection that arises is urinary tract infections 23 patients (45%), pneumonia 14 (27%) and sepsis 6 (12%) with most types of microorganisms are Staphylococcus coagulase negative for gram-positive bacteria (22%) and Acinetobacter baumanii for gram-negative bacteria (22%).
