Abstract

Background : Chronic obstructive pulmonary disease is an emergency situation that can cause death in patients where death from this cause ranks third in the world after cardiovascular disease and cancer. COPD patients have a 2- to 3-fold increased risk of sudden cardiac death. In COPD patients, the most common EKG features are bundle branch block. In this Final Scientific Work raises the case of Mr. S who has suffered from COPD (Chronic Obstructive Pulmonary Disease) with RBBB (Right Bundle Branch Block Block) symptoms. For this reason, this final scientific work aims to determine the effectiveness of pursed lip breathing on increasing oxygen saturation in COPD (Chronic Obstructive Pulmonary Disease) patients with RBBB (Right Bundle Branch Block) symptoms in the Rose Room, University of Muhammadiyah Malang Hospital.

Method : The research method used in the preparation of this Final Scientific Work uses case study reports. In this scientific work, data collection is carried out using primary data, namely data taken directly from the respondents. Data taken from research subjects included heart rate, respiratory rate, and SpO2 before and after pursed lips breathing intervention. In this method, the standard operating procedure for pursed lips breathing is to first position the client to sit comfortably, then relax the shoulders and neck. After that ask the client to close the mouth, then inhale slowly through the nose, until the chest and abdomen feel maximally lifted for 2 seconds. Mouth closed is maintained on inspiration and held for 2 seconds. Then exhale slowly with the lips closing or forming the letter O in a matter of 4 or 5 seconds. The intervention can be repeated within 10 minutes consisting of 5 minutes of intervention and 5 minutes of rest.
Results : After the client was given pursed lips breathing therapy for 4 consecutive days and the results obtained were Mr. S at the first meeting before being given pursed lips breathing therapy, SPO2: 94% room water and complained of shortness of breath. The client looks weak, the expiratory phase is prolonged, the nostrils are breathing. Then at the fourth meeting after being given pursed lips breathing therapy SPO2 increased to 98% room air and complaints of shortness of breath began to decrease, the expiratory phase was normal. The client says he is more relaxed and his breathing is not too tight anymore.

Discussion : Based on the results of the implementation that has been done, it shows that oxygen saturation before being given pursed lips breathing exercises to patients is 94% and after practicing pursed lips breathing oxygen saturation to patients is 98%. This is an increase in the oxygen saturation value in patients by 4%, meaning that pursed lips breathing exercises can increase oxygen saturation in COPD patients with RBBB (Right Bundle Branch Block Block) Symptoms.
