Abstract

Vendor Petrokimia Gresik a fertilizer and chemicals in Indonesia. Constraints in the company's product produced Phonska fertilizer grade 15-15-15, fertilizer in particular is still there are many defects in the production process. the types of quality criteria Phonska fertilizer is water content, N content, P content , K content, and grain size. To avoid the occurrence of defects and produce a quality product for customer satisfaction are met, then use Six Sigma methods to reduce these defects. Purpose of this study was to determine the quality criteria most often beyond the standard and its causes and propose improvements to reduce these defects. Stages include stage six sigma define, measure, analyze, improve and control. The first step is the preparation of diagrams and identification SIPOC, CTQ (Critical To Quality) and quality criteria, calculate the sigma level and process capability to determine the most influential of CTQ (Critical To Quality). Then find out the cause of the defect using a cause effect diagram, it is done after the identification of the causes of these defects by using FMEA (Failure Mode Effect Analysis). From the data processing is known that the quality criteria are most often beyond the standard N content and P content and after the known causes of these quality criteria by looking at the highest RPN in FMEA is given suggestions on improvements to reduce these defects. Proposed improvement is, that is by improving care at the scale 2 times in 1 month and pump treatment using the check sheet.
